1. Consignee or Notify:

• For all cargo: TAX NUMBER # COMPANY NAME # ADDRESS # TEL/FAX
• For Scarp/Waste Cargo: TAX NUMBER # IMPORT LICENSE NUMBER # SECURITY DEPOSIT NUMBER # COMPANY NAME # ADDRESS # TEL/FAX

2. HS Codes:

• For all cargo: HS Code with at least 4 digits
• For Scrap/Waste cargo: 8 full digits required

3. Description of Goods:

• Cargo Name # Package # Weight
